We are currently seeking a Psychiatric Nurse Practitioner to support the delivery of high-quality mental health services in an inclusive and trauma-informed environment.
Provide psychiatric evaluations, diagnoses, and medication management for a diverse adult patient population.
Collaborate with an integrated care team including primary care providers, therapists, care coordinators, and case managers to ensure holistic, patient-centered treatment.
Utilize evidence-based approaches and harm reduction models to support clients with co-occurring mental health, substance use, and chronic conditions.
Support continuity of care and contribute to the overall improvement of behavioral health outcomes across clinical sites.
Maintain accurate documentation in accordance with medical and regulatory standards.
Licensed as a Nurse Practitioner in the State of New York with a Psychiatric-Mental Health Nurse Practitioner (PMHNP) certification.
Experience working in community health, behavioral health, or with marginalized populations preferred.
Strong commitment to health equity, cultural humility, and LGBTQIA+ affirming care.
Bilingual (English/Spanish or other language) is a plus.
